what are tailored care plans?
Tailored care plans for individuals with intellectual disabilities are personalized, structured strategies designed to meet their unique physical, emotional, social, and cognitive needs. These plans are developed collaboratively by healthcare professionals, caregivers, and often the individual (when possible), and they focus on promoting independence, quality of life, and dignity.
Tailored Care Management
InReach provides Tailored Care Management (TCM) in Cabarrus, Mecklenburg, and Union counties. Tailored Care Management provides Tailored Plan members with a Care Manager
INnovation services
InReach provides a network of community-based service providers. Individuals who receive waiver funding work with their team to develop a person-centered plan of care and request the services and supports they need.
Who Qualifies for Tailored Care Management?
