Innovation Services

The Medicaid Innovations Waiver supports individuals with I/DD in living the life they choose. Waiver services are administered by a local management entity/managed care organization (LME/MCO), which facilitates services and oversees a network of community-based service providers. Individuals who receive waiver funding work with their team to develop a person-centered plan of care and request the services and supports they need.  

Residential

  • Residential Supports (in Alternative Family Living (AFL) homes and Group Homes) –  Provides services and supports to enable a person to live successfully in the home and participate in his/her community.
  • Supported Living – Provides a flexible partnership that enables an individual to live in their own home with supports from InReach.  Training activities, supervision, and assistance are provided in areas such as health and safety, budget management, social skills, attending appointments, cooking, shopping, etc.

Support Services

  • Community Living & Supports – Provides services that enable the individual to live successfully in his/her own home, the home of his/her family and be an active member of the his/her community.  Provides supervision and assistance in areas of monitoring a health condition, nutrition, incidental supervision, daily living skills, community participation, and interpersonal skills.  Enables the person to learn new skills, practice and/or improve existing skills in areas of interpersonal, independent living, community living, self-care, and self-determination.
  • Respite – Provides periodic or scheduled support and relief to the primary caregiver.
  • Supported Employment and Long Term Follow-up – Provides assistance with choosing, acquiring, and maintaining a job for individuals age 16 and older. 
  • Crisis Services – Provides intervention and stabilization for individuals experiencing a crisis.
  • Community Transition Supports – Provides initial set-up expenses for adults to facilitate their transition from an institution, ICF-IDD group home, nursing facility, licensed group homes, family home, or AFL to a living arrangement where the individual is directly responsible for his or her own living expenses.
  • Community Networking – Provides day activities that support the individual in a meaningful day in an integrated setting with persons who are not disabled. 
  • Community Navigator-Promotes self-determination, supports the individual in making life choices, provides support to individuals and planning teams in developing social networks and connections within local communities.  Community Navigator Services also emphasizes, promotes, and coordinates the use of generic resources to address the individual’s needs in addition to paid services.
